๐ Alaska Immunization Requirements
Under Alaska State law (4 AAC 06.055), all students attending school must be immunized or have a valid exemption on file.
Required Vaccines for School Attendance:
Students must provide documentation of the following immunizations:
- Diphtheria, Tetanus, Pertussis (DTaP/Tdap)
- Polio (IPV/OPV)
- Measles, Mumps, Rubella (MMR)
- Hepatitis A
- Hepatitis B
- Varicella (Chickenpox)
(or provider verification of disease history)
Additional Requirements:
- Students entering 7th grade must have a Tdap booster
- Immunizations must meet minimum age and interval requirements
Exemptions:
Alaska allows the following exemptions:
- Medical exemption (signed by a healthcare provider)
- Religious exemption (signed by parent/guardian)
Exemption forms must be submitted to the school and approved before attendance.
Important:
- Students who do not meet immunization requirements may not attend school until documentation is provided or an exemption is approved

Exemption Forms (State of Alaska)
Under Alaska State law (4 AAC 06.055), all students attending school must be immunized or have a valid exemption on file.
If your child qualifies for an exemption, please complete one of the official State of Alaska forms below:
Important Information About Exemptions:
- Alaska allows medical and religious exemptions only (no personal/philosophical exemptions)
- Religious exemption forms must be notarized and renewed annually (July 1โJune 30)
- Medical exemptions must be completed by a licensed healthcare provider
- Forms must be submitted to the school before attendance
Important:
- Students without required immunizations or approved exemptions may not attend school
- During a disease outbreak, exempted students may be excluded from school for their safety
